Nurse) or emailed to Laura..Prucyk@Colonial.k12.de.us Are your child's immunization records up to date? Not sure what your child may need? Check with your physician or contact us via email or during office hours to review the most current documentation the school has. HEALTH ACTION PLANS Does your child have a health concern that may require special attention at school? YES?? Please have their physician complete the appropriate action plan for this school year (available to download and print from this site) and submit it to the us via fax 302-832-6335 or email to Laura.Prucyk@Colonial.k12.de.us. **Note -- your student will need a new one each school year or before that if their condition/medication change.
When School Comes Back -- Please Send in EXTRA CLOTHES -- BE PREPARED! Please make sure your child has an extra set of season appropriate clothing (including underwear!) here at school. Accidents happen and whether it's a bathroom accident, spilled milk, or just falling in mud, it happens. It's always best to be prepared. Extra clothes can be sent in with the student's name and kept in the nurse's office. This helps reduce the calls to you requesting someone to bring in clean clothes in the middle of the school day and reduces time missed in class.
